Complete 6 Hours DMV certified defensive driving class no more than 6 months before filing the application Notarized Child Support Certification form from the NYC Office of Child Support Enforcement, form can be obtained within TLC location. Signed medical history form by a doctor proofing eligibility to drive a cab Money orders: $120 payable to N.Y.C. TLC $75 payable to N.Y.C. TLC/DCJS $25.50 payable to LabCorp
